AMETHYST HOUSE
RESIDENTIAL PROGRAM
APPLICATION PROCESS

APPLICANT: Please read this information for instructions about applying to the Amethyst House Residential Program.

1. Print out a paper copy of all forms from this PDF file. This is the copy that should be filled out. Return it and all other forms, as indicated.

You will need to have a Tuberculosis screen and physical exam (done within the past 90 days) before admission.

WHEN Amethyst receives your application an interview appointment will be scheduled with you; this can be done in person or over the phone.
FOLLOWING the interview and collection of all necessary information, the application will be evaluated by the Treatment Team to determine appropriate level of care.
Bed Availability: you should check in at least every other day. If Amethyst House has not heard from you within one week, your application will be removed from the application list.
Applicants that are incarcerated need to communicate at least monthly with Amethyst House.

If you are taking prescription medication(s), at least one month's supply is required upon admission. All medication must have a pharmacy label.

***Program Fees Amethyst House prefers that you have at least the first month's rent at time of admission.
