General FAQ:
Do you accept insurance?
- At this time, Amethyst House accepts the following insurance plans for Outpatient Services:
-If you are privately insured, we will make a copy of your card and have you complete a Release of Information for your insurance provider. Please note that this does not guarantee coverage; we are happy to contact your insurance provider to see if any co-pays apply and inform you of said co-pays. We can assist clients with the Medicaid enrollment process.
-Amethyst House receives subsidized funding from the state of Indiana for those who are income-eligible. This allows the vast majority of our clients to receive treatment services at an affordable, reduced-rate cost.
Are you an inpatient facility?
- No, Amethyst House does not provide inpatient services and we do not have medical staff on site at any of our locations.

Residential Program FAQ:
How do I apply for the residential program?
- Click here to access the application for our Men's and Women's Residential programs. This packet includes the Community Agreements, which outline the rules and expectations of our residential program. Completed applications may be returned to us by mail/fax, or dropping it off at out Outpatient office.
I have submitted my application for the residential program - what now?
- In addition to the completed application, we also require results from a TB test from within the last 90 days, a physical exam from within the last six (6) months, as well as records from any previous treatment providers. Amethyst staff can send for the treatment records, provided you complete and return a Release of Information (ROI) form for your treatment provider, which is included in the application packet. Please make as many copies of the ROI as needed if you would like us to be able to communicate with a spouse, family member, treatment provider, attorney, probation officer, etc.
- As we receive the required information, Amethyst staff will issue a response letter to you, indicating what information has been received and what information is still needed. Once we have all of the required documentation, we will contact you by phone or in writing to schedule an interview with our staff.
How long is the waiting list?
- This is one of the most common questions asked when applying for our residential program. Essentially, the length of our waiting list is always changing and what is more important is how quickly an applicant can provide us with all the needed information to conduct an interview. Assuming an applicant has been accepted into the residential program, we usually advise them that the wait for a bed can generally range anywhere from 4-6 weeks, but this is completely dependent on bed availability. We also offer preferential admission for pregnant women and IV drug users.
- Please note that our admissions coordinator makes every effort to answer phone calls and letters in a timely manner, but please allow 3-5 days for a response to waiting list/application-related inquiries.
- All application/waiting list inquiries should be directed to the appropriate house (Men’s: (812) 336-2612 / Women’s: (812) 336-2666). Please follow the instructions on our voice mailboxes as to where you should leave a message. Please leave only one message and speak clearly, providing your name and number. Due to our high call volume and busy staff, please allow 3-5 days for a response by phone.
Can I complete my interview over the phone?
- Unfortunately, we do not allow phone interviews. We typically conduct interviews at our men’s and women’s houses and we are also able to conduct interviews at our local jail and surrounding areas. Applicants are expected to provide their own transportation to the interview. If an applicant is incarcerated, Amethyst staff will work with the attorney, probation officer, etc. to make arrangements for a transport order.
What are the residential costs?
- Residential costs include room and board (i.e. rent, utilities, food) as well as weekly case management sessions. Please note that this does not include the costs of required outpatient treatment.
- For those clients that qualify for Medicaid, with a pre-authorization Medicaid will typically pay for an average of 30 days.
-We also have grant funding available for those that qualify to help with the cost of Residential treatment for approximately 30 days. After all outside funding sources are exhausted we have discounted rates for Residential treatment.
I’ve never done treatment before. Am I eligible for the residential program?
- Amethyst residents are comprised of all different backgrounds and experiences, with regards to substance use. Many of been through treatment before and others have not and are new to the AA/NA recovery community. For those who are considering our residential program, we highly recommend a undergoing treatment in a more intensive inpatient program first (see our Resources page for referrals), but we do not require this and will not deny anyone the chance to interview.
Outpatient Program FAQ:
How do I apply for the outpatient treatment program?
- If you are a resident at either our men’s or women’s houses, your case manager will help you schedule an appointment with our outpatient office. Intensive Outpatient Treatment (IOP) is a requirement
