Residential Services Application Instructions      

 
1. Access the Residential Application Packet and read the following sections:
  • Residential Application Process
  • Requirements for Admission   
  • Confidentiality Information             
  • Orientation Policy
  • Community Agreements  

2. Complete and submit the following forms only:
  • Application
  • Release of Information (as many as needed - 3 are included in the packet)

3. Mail, fax or drop off your application:

Once we receive your residential application you will be placed on our waiting list.  We offer preferential admission for pregnant women and IV drug users. While on the waiting list please submit a Tuberculosis screen completed within the last 90 days and a proof of a physical exam completed within the last six months. Most importantly please check-in by calling the appropriate house once a week to keep your application active.  

Once we have all the required documentation an interview appointment will be scheduled. Following the interview the application will be evaluated by our Treatment Team to determine if you are appropriate for the program.  

If taking approved prescription medication(s), you must have at least one month's supply upon admission.
